Expert Care from Nurse Practitioners

Nurse practitioners are integrated into our hospice team and provide inpatient care to residents. You’ll receive attentive care, medication and symptom management, acute care management, and discussions around goals of care.

Individualized Skilled Nursing

Each of our experienced hospice nurses works with a small number of residents, which means that residents and families receive more time and attention.

Social and Spiritual Care

Our dedicated social work team offers consistent support to residents and families based on their individual needs. Our hospice chaplain is part of the Oakwood Village Pastoral team, with a reputation for strong relationships with residents.

Direct Pharmacy Access

The Oakwood Pharmacy creates the fastest possible access to medications. Because the pharmacy is on-site, nurse practitioners and pharmacists can collaborate directly to best meet your needs.

Bereavement and Grief Support

Our caring team offers grief support in both individual and group settings. You can find more information on grief support
services by calling 608-230-4233.

Best-Practice Communication

We add hospice documentation directly into our charting system, creating seamless continuity of care across all staff.

Because our hospice team is part of the Oakwood community, they foster close relationships with both residents and staff. Many of our hospice volunteers are current residents dedicated to caring for their community.
